Smegma is a combination of exfoliated (shed) epithelial cells and sebum. It can be washed away with water. If you don't keep your foreskin clean it can become irritated. In the worse case, it can lead to an infection. Some boys do not have any foreskin because it has been removed due to religion or simple tradition
